New Patient Forms
Please arrive 15 minutes early for your first appointment to complete necessary paperwork.
Additional forms, such as the Medical Records Release Form, should only be completed if directed by our staff. You can download and print these forms ahead of time to bring with you.
Insurance and Payments
We accept most insurance plans—including Medicare, Anthem Blue Cross and Blue Shield, and many government programs—and will verify your benefits before your appointment.
We also accept checks and major credit cards, with financing options available through CareCredit to help make your care affordable.

Anderson Podiatry Center is committed to helping to heal the foundation of the human body. Founded by Dr. James Anderson in 1981 as Poudre Valley Foot and Ankle Clinic, Anderson Podiatry Center is now comprised of five practices, four offices and one surgery center. For years, Anderson Podiatry Center has been the only full-service foot care center in the tri-state area of Colorado, Nebraska, and Wyoming. Our compassionate, friendly doctors are skilled at finding the root of foot, ankle, and lower leg problems. Our podiatrists are passionate about improving the quality of life of our patients. We were the first doctors in the Rocky Mountain region to offer groundbreaking treatments for arthritis pain and neuropathy, and we are always looking for new ways to help our patients live life to the fullest. And our Podiatry Center now staffs medical doctors to provide exciting, effective, advanced treatments to the rest of the body for injuries and chronic pain, including regenerative medicine, neurostimulation and nerve resection surgery. Now there are new options that can actually reverse pain rather than mask the symptoms, and reduce or eliminate dependence on harmful drugs.
