may have regarding foot and ankle conditions, as well as chronic pain conditions
throughout the body. Below is some additional information to help your first visit go
smoothly.

Please plan on arriving 15 minutes before your first appointment to complete paperwork at
our office.

Payments
Our office accepts most insurance plans, including Medicare, Anthem Blue Cross and Blue
Shield, and many government plans. We will verify your coverage and benefits before your
first appointment.
We accept checks and most credit cards. We also offer financing through CareCredit.
